Based with the innovative Diabetes Specialist Nursing service, you will play a vital role in supporting the dynamic and evolving community intermediate diabetes service and diabetes specialist service. You will provide autonomous diabetes specialist nursing care within both community and Consultant-led diabetes services in a variety of settings. We are fully integrated with the wider diabetes specialist team and benefit from ongoing supervision and governance support. Main duties of the job

Within this challenging and mobile role, the post holder will work as part of an integrated diabetes multidisciplinary team providing diabetes specialist nurse care to people with diabetes in community and hospital based clinics. This support will include clinic attendance, phone contact and home visits. You will also contribute to the delivery and ongoing development of our locally developed structured patient education programme and our healthcare professional clinical quality education meetings. You will provide clinical leadership within the team and to other health care professionals working in hospital or community settings, providing mentorship for other team members and supporting the development and delivery of diabetes nursing services. Job responsibilities

You will be a registered nurse with a first level degree, have current and demonstrable experience of working within a diabetes specialist care team in a Band 6 Diabetes Specialist Nurse role, with a university accredited diabetes qualification. You will also be a non-medical prescriber or be willing to undertake the qualification. The post-holder will facilitate a smooth journey from admission to discharge for the patient with Diabetes admitted to a ward within Bradford Teaching Hospitals. The post-holder will have responsibility for a defined caseload and will provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers. Provide clinical, social and psychological support to their patients/service-users using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health. Facilitate in the development of the existing diabetes service, in line with local and National Guidance. Develop close working relationships with all members of the Diabetes integrated multidisciplinary team, based in both secondary and primary care. Person Specification

Significant post-registration experience, including experience of working in a relevant clinical specialty Experience of assessment, development, implementation and evaluation of programmes of care for patients/service-users/staff demonstrating knowledge of specific needs Experience of participating in clinical audit Experience in policy development Experience of developing innovative practice Experience of working in an MDT and across organisational and professional boundaries Skills
