Clinical services will be delivered in accordance with the health needs of the local population, ensuring that the level of care is appropriate to the candidate's experience and development. The programme will combine both supervised and unsupervised clinical practice across an allocated cluster of general practices, alongside structured university training and in-practice professional development. Over the two-year training programme, the post holder will develop and apply a wide range of clinical skills appropriate to the needs of the primary care service. These will include, but are not limited to: Health screening and promotion Cervical screening sampling Family planning and sexual health Immunisation and travel health Chronic disease management (including structured clinics and supporting self-management for patients with complex co-morbidities) The post holder will: Work collaboratively within the primary care team to assess, plan, implement, and evaluate nursing care that meets the needs of individual patients, appropriate to the nurse's developing knowledge and skills. Contribute to the achievement of local and national health screening and disease prevention targets. Provide high-quality, evidence-based care that is tailored to the individual needs of patients. Assist in the collection and analysis of data to evaluate the quality and productivity of services provided in the practice. About us

West Road Medical Centre , with time divided between this practice and others within the
West End Family Health Primary Care Network (PCN) . The West End of Newcastle is a diverse and dynamic community committed to improving health and wellbeing. Although the area faces challenges such as socio-economic disparities, it is supported by a range of health services and community initiatives designed to address these issues and promote better health outcomes. Collaboration between local healthcare providers, including general practices, public health programs, and community support networks, is central to improving access to care and promoting preventative health strategies for the residents. It is anticipated that the post holder will remain within the assigned locality throughout the programme; however, working arrangements may be subject to change, and the scheme reserves the right to alter them as needed. Primary Care provides 80 percent of all NHS Health Care and Primary Care Nurses are continuously extending their scope of practice and clinical skills to ensure that we are able to offer high quality, safe care to people with increasingly complex health care needs. Our programme offers successful applicants a 24-month individually tailored development programme. Applicants must have a BSc Honours Degree and the post holder will be expected to continue their education through training courses, university and independent study to enhance their knowledge and skills. Applicants must be: Educated to degree level Willing to develop a broad range of clinical skills Ambitious to continue to develop and progress their career in primary care beyond the life of the scheme In return, you will be allocated protected time to attend training courses and to consolidate learning within clinical practice. You will be supported throughout the programme by our lead nurse, clinical supervisor and the wider practice team. Applicants must be able to demonstrate understanding and appreciation of the different roles and responsibilities of nurses working in Primary Care. In addition, excellent interpersonal and organisational skills and the ability to work on your own initiative, as well as being an effective team member are essential requirements of this post.
