Croydon Health Services offers a truly integrated approach to paediatric nursing across our acute and community services, supporting seamless pathways for children and young people between the hospital and their home for those requiring short-term admissions to those with long-term health needs.

Key Responsibilities:

Manage children’s acute and long-term healthcare needs between the primary, secondary, and tertiary interface (case management supervised by the Band 7 Community Children’s Nurse as required). Work with and support families and other agencies, facilitating the delivery of nursing care. Facilitate effective communication links between primary, secondary, and tertiary healthcare providers, and the voluntary and statutory sectors. Serve as a children’s nursing resource for families and professionals, providing one-to-one and group teaching sessions as required. Have day-to-day clinical responsibility for the Children’s Hospital at Home Team caseload to meet the individual needs of children and their families in the absence of senior colleagues. Assess, plan, implement, and evaluate care, liaising and referring as necessary. Be responsible for all liaison, action, and documentation required following nursing care interventions.
