An exciting position has become available for a Band 4 Associate Practitioner to join the team on Tunstall Ward at Lanchester Road Hospital, Durham. The successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into the position on Band 4. Main duties of the job

This role is to support the urgent care pathway within the team. The successful candidate will work as lead professional carrying their own caseload of patients delivering psychological interventions within their scope of practice on a 1-1 and group basis. The successful candidate will take a lead role in group delivery and coordination. We will provide you with clinical caseload and managerial supervision, a thorough induction and an extensive range of training opportunities relevant to the role. Job responsibilities

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being. To be compassionate in meeting the needs of patients and their carers. To work as part of a community team which may be integrated, working collaboratively and in partnership with local authority services. To deliver patient care, as directed by registered practitioners and the team manager, undertaking a range of clinical/therapeutic interventions with limited or indirect supervision in patients homes and other community settings. To promote social inclusion, community access and participation through the provision of patient care. To show commitment to and participate in quality improvement activities. To promote at all times a positive image of people with mental ill health. To promote at all times a positive image of Health and Justice services and the wider Trust. Person Specification

Qualifications

Foundation Degree in Health and Social Care Key skills in literacy, numeracy and ITQ level 2 (or equivalent) Experience

Previous experience of working with people with mental/social health needs or learning disabilities Previous experience as a Band 2/3 HCA or equivalent in a care environment. Knowledge

Person centred care Evidence based knowledge of current practices in the field. Awareness of legal frameworks including Mental Health legislation Skills

Effective verbal and written communication skills Time management and ability to prioritise effectively Competency in venepuncture Personal Attributes

Compassionate in meeting the needs of vulnerable people and their families and carers. Able to work flexibly and co-operatively as part of a team Committed to continual quality and service improvement Self awareness and committed to continual personal development. Other requirements

Ability to travel independently in accordance with Trust policies and service need. Disclosure and Barring Service Check
