A rare and exciting opportunity has arisen for an Assistant Practitioner to join the existing Intensive Home Treatment Team (IHTT). We are looking for a highly motivated and enthusiastic person with experience of working with older adults with mental health problems. This is a multi-disciplinary service working in the Gedling and Hucknall localities. The service is for older people with mental health needs and is delivered in people’s own homes/care homes. The service works alongside the Community Mental Health Team (CMHT) and makes every effort to prevent unnecessary admission to hospital or residential care.

You will contribute to assessments and interventions, undertaking nursing and therapy assessment and intervention under the supervision of a registered clinician. The service is available 7 Days per week during the hours of 08:00 - 20:00. You will frequently work without direct supervision in the community and be required to work in a flexible and holistic way in order to respond to this client group’s needs.

This role requires working across differing professions including nursing, occupational therapy, physiotherapy, and social work. You will undertake assessments and interventions which are competency-based, working in a flexible and holistic way to meet the needs of this patient group. You will take an active role in the development and supervision of community support workers, as well as contributing to the development of the service.

Responsibilities will also include supporting with the community support workers' rotas and allocating daily work.

Key Responsibilities

To visit service users in their homes. To provide assessments, interventions, and reviews in a person-centred and flexible way. To ensure robust risk assessments are completed. To recognise limitations and refer to other team members as appropriate. To work independently and manage own work/caseload. To respond appropriately to urgent and non-routine situations. To ensure accurate record keeping and completion of electronic recording requirements. To ensure confidentiality is maintained at all times. To contribute to reviews and accurately report back in a timely manner. To provide support and training to other team members as required. To participate in non-clinical team meetings in order to contribute to the development and effectiveness of the service. To actively contribute to the development of the Assistant Practitioner role. To undertake any other reasonable duties as requested by the Team Leader.

Health and Safety

To work within all Trust guidelines in relation to infection control, manual handling, lone working, and fire. To report to clinician in charge any untoward incidents or near misses.

Education and Development

To identify with clinical supervisor training and development needs. To participate in mandatory training and developing of nursing and therapy related competencies. Ability to recognise own limitations and seek appropriate support.

Please note that this post does not meet the pay level required for a Skilled Worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances, the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
