The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users, supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate. Working under the supervision of the Deputy Team Manager/Team Manager, the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery-focused and developed in collaboration with the service user and, where appropriate, their relatives and carers.
