An excellent opportunity has arisen to recruit a Social Worker to North Manchester Later Life Community Mental Health Team. The experienced recruit will operate within an integrated, multi-disciplinary team and will mainly undertake commissioning and care purchasing tasks for service users, working within the framework of the Mental Capacity Act 2005. Overview: Mental health services in Manchester are commencing their journey around Community Transformation. Working towards the new Community mental health framework, the vision is for a place-based community mental health model. Social work is critical in upholding individuals' human rights, fostering a strengths and asset-based approach to an individual's mental health recovery. We are looking for social workers who take an anti-oppressive approach to social care interventions. Social work expertise is a key component to multi-disciplinary integrated working. We have roles which will enable you to carry out specific social care duties for individuals. Main duties of the job: The successful candidate will also complete assessments for clients and carers and at times also provide care coordination in line with the Trust Care Programme Approach (CPA) guidance and policies. You will be allocated care coordinator/lead professional for a caseload of both organic & functional clients. You will be expected to participate in the rostered Duty role which includes initial CMHT assessments of new referrals and feeding these back at the Multi Disciplinary Team Meetings. Complete Safeguarding processes as needed with support of Team Manager and Seniors. The Community Mental Health Team operates from Monday to Friday during usual office hours.
