To support the Clinical Site managers to provide a 7 day service 365 days per year.

To manage and support the Administration for the Clinical Site Team and the point of contact for all administrative issues relating to patients' pathway for emergency care.

To support the Clinical site managers in ensuring the effective and efficient utilisation of all acute beds within the Trust, with due regard to the balance between the admission of emergency and elective patients and ensuring flow with timely discharges (especially those that require transport).

Main duties of the job

To facilitate bed allocation for all specialities by ensuring that patients are placed into the most appropriate speciality bed and in doing so support the Trust to meet national targets. Engaging with all areas that impact patient flow across as required. To have on-going awareness of patient flow issues e.g. bed availability, current bed state, outliers, and bed closures. Be the point of contact for all administration relating to patients' pathway for emergency care within the Site Team. Work as part of a multi-disciplinary team and provide proactive management of the patient pathway from referral to admission, transfer to wards, ensuring the entire pathway is managed smoothly and discharges and transport are managed to support flow. To provide training for all Trust staff on how to book the correct transport for all patients that require transport for discharges.

To work with the Clinical Site Manager to develop expertise and effective high quality patient flow systems, training for all staff re transport bookings for effective discharging. To support the repatriation of patients from other trusts and abroad to the Trust in line with the repatriation policy. To follow the system of escalation regarding bed status. Supporting the Lead CSM with transport datix and collecting data for reporting for performance review and for Clinical Governance.

Role Responsibilities

To manage and support the administration for the Clinical Site Team and be the point of contact for all administrative issues relating to patients pathway for emergency care and ensuring all staff have appropriate training for booking transport to ensure correct transport booked for timely discharges. Ensure appropriate and effective allocation of beds for both emergency and elective admissions. The aim is to ensure patients are allocated to the right bed for their care. Understand the workload targets, which have been set for clinical areas, and demonstrate commitment to their delivery. Ensure that quality and risk management issues are escalated appropriately. To have ongoing awareness of capacity issues e.g. Bed availability, current bed status, outliers and bed closures. To utilise the escalation policy and cross care group transfer procedures appropriately. To effectively communicate the bed state to key personnel (ie On call manager, Clinical Site Manager, directors etc). To take every possible step to ensure the Trust meets local and national standards i.e. single sex accommodation, 4hr access target, ambulance handover and 18 week target. To prioritise competing demands in relation to utilisation of capacity. Contribute to service and policy development in relation to Trust bed management and transport services and bookings. Ensure patients are being transferred from other hospitals or are being repatriated to other hospitals is co-ordinated in a timely, efficient manner and against local and national targets. Assist in the monitoring and maintenance of the health, safety and security of self and others in work area. Identify and assess risks in work activities, and assist in developing strategies to manage these effectively. Ensure team works within risk management/ occupational health legislation and Trust procedures. Contribute to the implementation of the development of the service. Be aware of new evidence and research in both general and specialist fields. Work creatively to develop practice and new ways of working, taking the lead in the change management process when appropriate. Develop own skills, taking responsibility for continuing professional development and performance, maintaining own portfolio. Works to achieve competencies appropriate to grade. Is able to identify own development needs in relation to current practice and future plans, setting personal development objectives.
