This is an exciting role for a kind, enthusiastic, experienced nurse who is keen to join our cardiac nurse team at Harrogate District Hospital. We are looking for someone with cardiology experience both in hospital and community.

We are looking for someone who is keen, in addition to Cardiac Rehabilitation, to have an interest in supporting inpatients with heart failure and liaising closely with the community Heart Failure Specialist Nurses.

Main duties of the job

To deliver cardiac rehabilitation to patients from the Harrogate and Rural District in both the hospital and community as part of the integrated cardiac rehabilitation team. To provide support and education for patients admitted with heart failure with complex health needs to Harrogate District Hospital. To provide support and education to patients admitted with arrhythmia to Harrogate District Hospital.

Job responsibilities

To assess, plan, implement, and evaluate a programme of Cardiac Rehabilitation as per NICE Guidelines and in line with BACPR standards to cardiac patients in primary and secondary care across the Harrogate and Rural District. To work independently and in conjunction with other members of the team to deliver this high-quality service to patients following cardiac surgery, ACS, or PCI through face-to-face consultations and by telephone both in the hospital and in the community. To support in-patients with heart failure with often complex health needs in Harrogate District Hospital in terms of information, education, management, and onward referral to the community heart failure nurse team.

Person Specification

Essential and desirable

Essential

NMC registration Educated to degree level 2 years post-registration experience with some cardiology nursing experience Full driving license and use of a car for community work in order to meet service needs

Desirable

Medical prescribing - willingness to work towards Degree modules in Cardiac rehabilitation, cardiac care, and heart failure Experience of teaching and presenting at meetings Advanced knowledge of cardiac conditions
