The post holder will be responsible for key working (under supervision), a caseload of clients aged 65 years and above, with both functional and organic illness. There is a Duty service provided by the team Mon-Fri 9-5pm, and the post holder will participate in our triage process, working carefully to ensure our clients are directed to the right support they need after referral to our team. There will also be an exciting opportunity to undertake some Memory Assessments and to become involved in our Liaison Service linking in with Weston General Hospital, Bridgwater Community Hospital, and Burnham on Sea Community Hospital. We are keen to support our own clinicians with professional development, and the successful post holder will have the opportunity for further training and will be encouraged to participate in supporting students and supervising other staff.
