Full-Time or Part-Time Hours Available Two-Year Fixed-Term Contract We have an exciting opportunity for a Palliative/End of Life Frailty Lead to join our team at St Luke's Hospice working predominantly in the community setting. This fabulous opportunity is to help support the delivery of outstanding quality care to frail and vulnerable people within Plymouth and surrounding areas, supporting patients with recognised moderate to severe frailty and could be within the last year of life. You will use your experience and specialist palliative / end of life care knowledge to work closely with allied health professionals to deliver appropriate management, advance care planning, undertake comprehensive geriatric assessment and medication review for end-of-life care. As part of our commitment to a continuous improvement strategy for this group of individuals and, in order to reduce variation in care with the focus being on standardising practice we have created this innovative and autonomous post. The Palliative/EOL Frailty nurse / AHP role will be pivotal in achieving this aim by leading the way. This is a new role within St Luke's Hospice and will be pivotal to the delivery of a new pathway and model of care for the frail and elderly who are in their last year of life and approaching end of life. The successful candidate will ensure that the frail person is at the centre of decision making across all settings, ensuring that the risk of these patients being admitted to hospital unnecessarily is reduced. A further aim of the role is to work towards a model that ensures only those patients who require hospitalisation are admitted and discharge is proactively expedited to their preferred place of care. The role will require a collaborative approach working alongside our partners within Plymouth and Devon who care for the frail and elderly including nursing homes, care homes, district nurses, and GPs.
