What you will do:

Manage caseloads effectively on Elderly Medicine wards, addressing multipathologies and fast-paced patient turnover. Engage actively in team meetings, attending handovers and planning your day efficiently. Conduct frail elderly assessments, problem-solve, and execute interventions alongside discharge planning tailored to patients' needs. Identify and provide necessary equipment, ensuring patients have resources for best recovery outcomes. Collaborate seamlessly with Occupational Therapy teams, delegating tasks to assistants and liaising with MDT, carers, and families. Complete necessary statistics and data, accessing ExtraMed and making referrals to community services as required.

You must be HCPC registered and have firsthand experience working in the NHS, particularly as a Band 6 Physiotherapist on Acute Medical or Elderly Wards.
