The postholder will be required to undertake: Assessment of patients requiring home parenteral solutions, including choosing the most appropriate intravenous access device. Understand the principles of normal intestinal function and how gut function has changed in someone with intestinal failure. Minimize the effect of GI changes on fluid balance. Management of intravenous access device for parenteral nutrition (peripheral midline catheter, peripherally inserted central catheter (PICC), skin tunnelled catheter, and implantable port). Troubleshooting complications related to parenteral nutrition administration, IV access complications, and intestinal failure issues. Referring to other Health Care Professionals for support and advice in managing complex patients. Assessment of a patient requiring urgent review of their nutrition/hydration status, or management of a complication associated with artificial nutritional support. Staff, carer, and patient training and education across the trust and in the community. Ensure annual peer review and appraisal of Health care professionals in intravenous access management and intestinal failure management. Supporting and providing information for patients requiring nutritional support. Professional Responsibilities
Develop close working relationships with relevant multidisciplinary teams. Contribute actively to the Directorate's and Trusts' clinical governance work. In collaboration with colleagues, develop new ways of managing the service to meet patient needs. Maintain key performance indicator (KPI) data and a database of home parenteral nutrition patients to assist in audit and management of disease in patients referred to the Home Parenteral Solutions Group. Develop effective communication with CCG's and other professionals working in the community. Work with the Matron Gastroenterology Care Group to develop a comprehensive Public and Patient Involvement Strategy. To work at all times within the regulations of the Nursing and Midwifery Council. To work within Trust policies, protocols, and guidelines. Work within Guidelines published by NHS England in relation to the HPS framework. To maintain competency in intravenous access device placement and other skills practised. Research and Audit
As part of the multi-disciplinary team, undertake research and audit to improve the service and treatments for patients. To promote effective collaboration between practice, research, and audit from a departmental and Trust perspective. Lead and contribute to practice-based research with the Matron Gastroenterology Care Group. Education
Training and education will be provided in all skills covered. The post holder will be expected to develop and maintain up-to-date knowledge, skill, and competence to meet the needs of patients and carers. To successfully complete relevant training courses. To undertake further training and formal education to meet the developing needs of the role. Educate and develop practice of colleagues and junior staff.
