An opportunity has arisen in the Integrated Autism and ADHD Service within Powys Teaching Health Board. This is an exciting, busy and challenging time and we are looking for a dynamic and enthusiastic Assistant Practitioner to work within the team. The post holder will work with the integrated multi-disciplinary team to develop and deliver support interventions and also facilitate some group programmes. The post holder will be expected to develop and implement designated portfolios under the supervision of the clinicians within the team. The post holder will need to be competent using technology and will be expected to engage with people over the telephone and via video conferencing as well as face-to-face sessions. Experience of working with adults who have neurodevelopmental conditions is highly desirable, as is experience of facilitating groups and teaching. A willingness to work with the rest of the team to develop innovative approaches to help this group is essential. The post requires attention to detail, enthusiasm, flexibility, and resilience to work with a developing forward-thinking service. The ability to communicate effectively and build rapport and effective relationships at all levels is essential. The post holder will ideally have worked in a Partnership arena previously as well as the community. Ability to meet travel requirements for the post is essential. Main duties of the job
Independently manage own work and case load, undertaking tasks delegated by a Registered Practitioner with appropriate supervision in place. Understanding of evidence-based practice and delivering support in line with current evidence, take responsibility for acting relative to an individual’s health and care in accordance with organisational policy and procedures. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Delivers high quality, compassionate, evidence-based support to a group of clients following the initial assessment by the Registered Practitioner utilising person-centred approaches which promote health and independence. Under leadership and direction of the Registered Practitioner ensure that all clients have appropriate individualised, clinical assessments/risk assessments providing accurate feedback to the team as necessary. Effectively monitors the clients condition recognising deterioration in their physical or mental condition and liaises with the Registered Practitioner in an appropriate and timely manner to review/reassess support needs as required. Contributes to the on-going assessment of the client providing information to inform assessments and discussing any changes to planned care with the Registered Practitioner. Person specification
