You will work in the Hospital Discharge Mental Health Team. This exciting opportunity offers a competitive annual salary of £44,711 for a 37-hour workweek on a permanent basis. What you will do: Deliver strengths and assets-based social work services in line with the Care Act, Mental Capacity Act, and Mental Health Act. Collaborate closely with Herefordshire & Worcestershire Health and Care NHS Trust to deliver specialist mental health services. Use the Three Conversations approach to focus on clients' strengths, personal assets, and community resources. Conduct assessments, create support plans, provide interventions, and evaluate outcomes effectively. Handle safeguarding enquiries and work adeptly in complex situations using professional judgment and a range of interventions.
