The post will involve holistic mental health assessments, signposting and liaison, specialist clinical input to children and young people with physical health and co-morbid mental health issues (e.g. eating disorders), working with parents, schools, and educating colleagues. You would need to be confident and competent in positive risk assessment and management with robust care plans and be able to provide brief solution-focused interventions to support children, young people, and their families. You will undertake urgent assessments of people with mental health problems in acute crisis and, where appropriate, provide intensive community-based treatment/support services as an alternative to inpatient admission. You will also work with partner inpatient agencies to support timely discharge from hospital back to the community.

You will be part of a robust multidisciplinary team (MDT) and attend regular clinical caseload reviews, as well as completing any associated tasks to ensure safe care and/or discharge planning. You will be required to keep clear and concise records on young people under our care.

To be a point of contact for young people and families as crises arise, you will be expected to be able to employ effective de-escalation skills and to ensure that the young person is referred or signposted to the most appropriate level of care.

You will be responsible for ordering and monitoring the efficacy of prescribed medications and providing feedback to the MDT.

The service is available 365 days a year. Please note that this role is on a shift basis (long days) and weekends, but does not include night working.
