Formulating treatment plans to provide a holistic service to meet mental health and physical health needs. Completing comprehensive assessments, including assessment and management of risk. Undertaking urgent medication reviews for those in crisis. Exploring alternatives to admission and advocating for intensive community-based/home treatment options. Providing advice, support, and guidance where required to the MDT. This role involves largely community visits within the urban/rural areas of West Suffolk. Supporting staff development and acting as a role model to the team, demonstrating the behaviors required in line with our Trust values. Participating in Quality and Safety Reviews within the Trust. Undertaking Line Management Supervision within the team for an agreed number of staff.
