Deliver expert pharmaceutical care to patients in a GP practice setting. Lead chronic disease management, including Asthma, COPD, and Diabetes. Perform medication reviews and optimise treatments for better outcomes. Collaborate with GPs, nurses, and other healthcare professionals to deliver holistic care. Educate patients on medication usage, potential side effects, and disease self-management. Requirements:
Primary care experience is
essential . Proven experience managing chronic diseases like Asthma, COPD, and Diabetes. A recognised pharmacy qualification. Strong communication and teamwork skills. The ability to work autonomously within a multidisciplinary team. How to apply:
