Prison located in Redditch. In this role you will have the opportunity to provide the highest standards of healthcare to our patients, without having to compromise on efficiency and quality. Hours

Monday to Friday: 08:30 - 16:30. There will be an expectation to work some weekend and night shifts, as required. Salary

This position is full time, you’ll receive an annual salary between

£41,094 - £47,466

per annum - depending on experience. How will we support you?

What you'll be doing

As a

Planned Care Lead , your responsibilities will vary. You will: Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admission. Work with Head of Healthcare and Deputy Head of Healthcare to develop processes and methods to treat, inform and guide patients through their treatment, recovery and long term goals. Provide visible leadership, being accountable for the quality and assurance agenda, continually reviewing care and identifying areas of learning within your team. Introduce and embed new ways of working through innovation and respond to new service requirements, whilst continually enhancing care provided to our patients. If you are interested in

Qualifications & Experience Required

NMC Registered General Nurse or Paramedic with current HCPC registration Evidence of Continued Professional Development Significant post registration experience in clinical primary care practice Experience managing teams within regulated environments Understanding of the complexities of multi-disciplinary and multi-agency working Secure environments are one of the most challenging, yet rewarding places for healthcare professionals to work.
