The post holder will be part of a rotation between the Home First/Reablement team, the Core team and the Referral Centre team, all within Gloucester ICT. The post holder will work as an integral member of the Integrated Community Team to provide a service through assessing, collaborative goal planning, delivering and evaluating treatment for adults over the age of 18 with a physical disability as follows: Using recognised models of practice and evidence-based interventions and outcome measures to underpin all practice. Assessing and treating own caseload of patients/clients, adopting a health coaching approach for all interventions. Providing supervision and support to OT Assistants, Students and Reablement colleagues as appropriate. Providing occupational therapy advice to other disciplines and promoting the role of OT within the wider team. Participate in departmental research/project work when opportunities arise. Main duties of the job

Carrying out evidence-based assessment, intervention and evaluation of occupational performance needs of patients with physical disabilities. Be competent in risk assessments and managing risk autonomously within the home environment. To advise other professionals e.g. district nurses, carers. Liaise with partner agencies to ensure patients access to environmental adaptations via the Disabled Facilities Grant process. To manage a diverse caseload of patients with complex needs. Understand the nature, aetiology and expected progression of a wide variety of diseases/disabilities and to plan appropriate interventions. Understand the psychological and sociological impact of disease/disability on patients and carers. Provide training, advice on lifestyle changes and adaptations to the physical, psychological and social environment to meet the occupational performance needs of the patient. To plan and implement individual interventions by setting goals in collaboration with the patient. Attend appropriate specialist courses and cascade training to colleagues. To monitor and support supervisees e.g. Occupational Therapy Assistants, students. Person specification

Qualifications

HCPC registered Occupational Therapy qualification Experience

Ability to demonstrate judgement in complex clinical/operational problems, which require investigating, analysing, assessing and resolving. Awareness of the application of Models to address Occupational Performance issues. Ability to monitor and manage own and teams performance. Ability to deliver uni/multi-disciplinary training. Experience of working as a registered Occupational Therapist. Staff supervision. Specialist training or equivalent competency in environmental adaptations through prior experience. Moving and Handling experience. Experience or training in a rehabilitation approach utilising patient-centred goals to maximise function.
