As an ACE Early Years Practitioner and Care Coordinator, you will work as part of the ACE health and wellbeing team to champion and deliver the adult ACE's recovery toolkit programme. Main Duties of the Job
Maintain and develop quality Creche provision sensitive to the diverse needs of parents and children affected by adverse childhood experiences. Work in partnership with families to support parental understanding of their role in their child's development. Identify levels of risk, vulnerability, and needs, and manage risk effectively. Provide a trauma-sensitive, empowering, person-centered approach. Sensitively challenge children, families, and individuals. Implement safeguarding procedures and the escalation policy. Recognize factors impacting families such as domestic violence, abuse, mental health difficulties, substance misuse, poverty, and housing issues. Work well in a multi-disciplinary team. Organize and manage time effectively, including planning, prioritizing, and record keeping. Ensure data integrity, confidentiality, and security of personal identifiable information. Exhibit excellent interpersonal and communication skills with diverse backgrounds. Commit to equality, diversity, and inclusion. Prepare resources for groups and Creche. About Us
South Sefton PCN is a large Primary Care Network covering four localities in south Sefton with a population of around 155,000 residents. PCNs are groups of GP practices working with various partners to improve health outcomes. South Sefton PCN Mental Health Hub provides proactive primary care level mental health and psychological support to residents. Job Responsibilities
The Care Coordinator will support the delivery of high-quality patient care and provide a safe, nurturing, and responsive environment for young children in a trauma-sensitive crèche setting. Work with GPs and the PCN team to identify and engage clients benefiting from early help. Maintain and develop quality Creche provision sensitive to diverse needs. Deliver targeted activities in groups addressing root causes utilizing the Rockpool Recovery toolkit programme. Support individuals and their carers, coordinating across health and care services. Signpost participants to community activities to build resilience. Help manage needs by answering queries and maintaining accurate records. Assess and identify levels of risk and safeguarding. Support assessments via phone and face-to-face in the community. Work co-operatively with colleagues and partners to share information. Ensure compliance with quality assurance, legislation, and policies. Engage in service development and continuous professional development. Utilize marketing techniques to promote service awareness. Ensure client information is gathered lawfully and accurately. Person Specification
Qualifications
Grade A to C in English and Maths or equivalent. Rockpool ACE recovery toolkit training. Qualified to NVQ level 2 in Health and Social Care or relevant experience. A current driving license and access to a vehicle. Skills
Skilled in maintaining quality Creche provision. Able to identify and manage risk effectively. Ability to provide a trauma-sensitive, empowering approach. Broad knowledge of safeguarding procedures. Effective communication and organizational skills. Knowledge of data integrity and confidentiality. Excellent interpersonal skills. Experience
Practical experience in delivering support to individuals and families. Experience in childcare provision settings. Experience in a relevant field or industry. Understanding of the wider determinants of health. Previous experience in NHS settings or GP practice. Experience working as part of a multi-disciplinary team. Disclosure and Barring Service Check
