Adult Nurse Case Manager (Tracheostomy Experienced) , you would be responsible for providing face-to-face support to our clients and their families whilst creating personalised care plans for each client. You would work closely with a core team of carers, training and supporting them with clinical skills and activities. This is a great chance to be part of a growing business that delivers advanced care and improves the lives of clients with the following conditions: Spinal cord injuries Neuromuscular disabilities Tracheostomy / ventilation Acquired brain injuries Learning & physical disabilities Key Requirements: Registered Nurse with a minimum of 12 months’ work experience in the UK and an active NMC PIN Tracheostomy and ventilator experience desirable ITU/HDU experience would be desirable Care of the spinal patient knowledge desirable Enteral feeding experience essential Ability to work flexibly and respond quickly to changing priorities whilst meeting deadlines Ability to build strong relationships with clients, their families, and carers Supportive management style that can bring out the best in others Confident, passionate, and possess a “can-do” attitude Ability to manage your own caseload of community packages which includes working in the community and at Wimbledon head office
