We are currently in search of a committed individual to join our Adult Social Care, Care Homes Team within the Living and Ageing Well service. We are looking for passionate individuals to conduct Care Act Assessments, Mental Capacity Assessments, Best Interest Decisions, and follow Safeguarding procedures for vulnerable older adults living with dementia. Key responsibilities : Developing relationships with care homes Collaborating with other professionals Ensuring the well-being and quality of life for our residents If you have experience in social care, a compassionate nature, and a drive to make a positive impact in the lives of older adults, we want to hear from you.
