As part of this team, you will be pivotal in supporting younger adults with physical disabilities and sensory impairments, as well as older people, to live as independently as possible. Your expertise will help coordinate care through annual reviews and offer guidance during transitions from children's to adult services. What you will do: Develop and maintain strong working relationships with partner agencies to provide seamless support for individuals. Conduct assessments under the Care Act 2014 and provide care management for younger adults and older people. Facilitate hospital discharges and manage safeguarding enquiries, ensuring service users' well-being. Coordinate and lead on annual reviews of support plans and manage direct payment work. Support young people transitioning from children's to adult services, ensuring their evolving needs are met. Work as a duty worker on a rota basis, providing essential support to the team. Requirements of the Social Worker: Degree or equivalent in social work Current Social Work England registration Significant frontline experience in Social Services within the UK working as a qualified Social Worker Living and working in Bromley is a fantastic choice because of its rich community spirit and the perfect balance it offers between city excitement and suburban tranquillity.
