You will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. We will put you in the best possible position to assess and review the social care needs of our service users and their carers. You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control. Within the role, you may work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments, people with a learning disability and people with mental health problems. Promoting independence, choice and control is at the heart of this position. This is an exciting opportunity to work with people to maximise their potential for independence. Applicants will enjoy working with people and have experience of direct work with the public as part of paid or voluntary work, although they may have limited or no experience of visiting people in their own homes or care settings. Shortlisting Criteria Your understanding of Adult Social Care needs and any experience you may have in this area. It doesn't necessarily have to involve visiting people in their homes or in care settings Your ability to manage a caseload efficiently, regularly monitoring support plans for individuals and carers Experience of working directly with the public on the phone or in person Your ability to build relationships with various stakeholders, colleagues, and the public effectively Your IT skills with a willingness to learn to use a variety of software systems Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.
