What you will do: Provide top-quality social work services to adults in the Bailiwick of Guernsey with a special focus on the Rapid Response and Reablement caseload. Conduct thorough assessments using standardised formats and coordinate services for those wishing to remain in their own homes. Lead as the primary Social Worker within the team, offering expert guidance, support, and potentially managing social work staff. Formulate and assess complex psychosocial needs, contributing to holistic assessments as part of a multi-professional team. Offer support, advice, and signposting to service users and their families on housing, benefits, and community services. Respond swiftly to referrals and hold a caseload of complex cases, providing person-centred support and respecting user dignity. Collaborate closely with multidisciplinary teams in meetings and case conferences to ensure comprehensive care planning.
