Adults Social Worker Team: Community Mental Health Team Location: Bolton Rate: £30 Bolton requires a Social Worker with CMHT experience, and ideally candidates should have Paris or Liquid Logic experience too. This is an exciting opportunity for Social Workers to join the Community Mental Health Team in Bolton that provides a comprehensive multi-disciplinary service. The Bolton CMHT is an integrated Health and Social Care service that strives to deliver a recovery-focused, trauma-informed, needs-based service for people with severe and enduring mental health conditions. They are a friendly and supportive MDT team who value working with people from diverse backgrounds and cultures. The Functional CMHTs are part of the Bolton Division of Greater Manchester Mental Health NHS Foundation Trust and provide a comprehensive multi-disciplinary service to people aged 18 years and above. They therefore welcome flexible and creative working to meet the needs of the people we support. As a Social Worker within the Bolton CMHT you will assess service users using identified assessments, risk assessments, and undertake the role of Care Coordinator in line with the CPA process. You will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway and will be expected to work proactively and flexibly within a multi-disciplinary team model.
