Full-time Main duties of the Social Worker: Lead on annual reviews of support plans, extra care housing support and direct payment work Provide assessments under Care Act 2014 and work with young people transitioning from Children’s services to Adult services, and beyond Support Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed Work as a duty worker on a rota basis, facilitate hospital discharges, undertake Safeguarding (s42) enquiries, undertake Mental Capacity Assessments/ Best Interest decisions Undertake face to face visits to families Requirements of the Social Worker: Degree or equivalent in social work Current Social Work England registration Significant frontline experience in Adults Social Services within the UK working as a qualified Social Worker This Social Worker job is advertised by Ashley McSheen.
