About the Role/Responsibilities: This is an opportunity to join a newly established Birth to Settled Adulthood Service, working with young people and their families to support them as they approach adulthood. You will be part of a full-time team working 37 hours, in a strength-based multi-disciplinary way across health, social care, SEND, and education. The role has a focus on preparation for adulthood from the age of 14 to 25 years. You will be completing Care Act assessments and care and support plans for young people along with other associated activities, such as completing MCA assessments/Best interest meetings. You will also take a role in supporting colleagues with all aspects of preparation for adulthood work such as developing independence across all ages. You will be part of a peer group of professionals who are committed to proactively supporting Dorset young people and their families to achieve a smooth transition in a prepared and informed way without any gaps. This role will cover East Dorset. Your base can be agreed depending on where you live and what works for you, with an opportunity to combine with working from home. Required Skills and Qualifications: Enhanced DBS for Children's and Adults (we can obtain for you) Eligibility to work in the UK SWE (HCPC) Registered Degree in Social Work (or equivalent) 2 years post-qualified experience within the UK (ideally) Strong understanding of the needs of children and young people with SEND and disabilities. Excellent communication and interpersonal skills. Ability to work collaboratively with various stakeholders and partners. Commitment to improving outcomes for children and young people.
