Advance Nurse Practitioner - Enhanced Care Home Team

This is a new exciting opportunity for the right individuals to join a service that supports the provision of the Enhanced Health in Care Home aspect of the National DES PCN Contract. This service reaches across a large proportion of the Care Home Sector within Dudley, providing first-class health care to residents who fall under the scope of this new service. The vision for this service is to be regionally leading and improve the health care offering to the care home population, enabling a more proactive health care vision with prevention and admission avoidance at the heart of the delivery model. The aim of the Enhanced Care Home Team is to support the provision for the "Enhanced Health in Care Home DES," working in a small team of MDT health care professionals to provide this service across approximately 40 care homes within the Dudley Borough. Main duties of the job

Assess, plan, develop, implement, and evaluate treatment programmes that promote health and wellbeing. Undertake duties associated with the Enhanced Health in Care Home (EHCH) National DES Requirements. Manage a patient caseload and build relationships within allocated care homes in the Dudley locality. Be confident in independently prescribing and reviewing medication, appropriate to patient needs and in accordance with evident practice and national and practice protocols. Triage visits and provide telephone advice as appropriate. Deliver care in line with NICE guidelines and evidence-based care, identifying patients who are entering the final months of life using the Gold Standard Framework (GSF) proactive identification guidance. Co-ordinate the planning and delivery of care, ensuring patients and carers/relatives are involved. Diagnose and manage acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan. Request diagnostic investigations and interpret within agreed protocols (e.g., x-ray, blood tests, scans, etc.). Assess, diagnose, and treat patients/residents within care homes who require medical attention and/or refer to appropriate agency if/as required. About us

Qualifications

Active 1st Level Registration with Nursing and Midwifery Council. Non-Medical Prescriber qualification (V150/V300) or working towards. Experience in diagnosing and managing minor illness. Working in a GP or Community Setting. Skills & knowledge

Ability to work within own scope and independently. Management of Long Term Conditions/Complex needs. Competent or working towards completion of competencies identified in the RCGP - Core capabilities framework for Advanced Clinical Practice working in general practice. Ability to record accurate clinical notes. Experience

Knowledge of EMIS Web and DOCMAN. Minimum of 2 years' experience working within healthcare settings. Experience in diagnosing and managing minor illness. Clinical examination skills. Disclosure and Barring Service Check
