Provide strong leadership and management to the CNS team ensuring high standards of patient-centred care are consistently delivered. Act as a key figure in the development and enhancement of palliative care services, fostering innovation and ensuring services align with local and national guidelines. Strengthen integration across health and social care boundaries, working with partners to deliver seamless and effective care for patients and families. Serve as a role model for clinical and professional excellence, maintaining a balance between direct patient care and strategic responsibilities. Promote a culture of learning and excellence through education, training, and reflective practice. To develop as a Clinical Practitioner and contribute to the on-call rota. About us

Job Purpose: As the Advanced Clinical Nurse Specialist, you will: Provide strong leadership and management to the CNS team, ensuring high standards of patient-centred care are consistently delivered and aligned with best practice guidelines. Act as a key figure in the development and enhancement of palliative care services, driving innovation and ensuring services adhere to both local and national standards. Serve as a role model for clinical and professional excellence, maintaining a balance between providing direct patient care and fulfilling your strategic responsibilities. Promote a culture of learning and excellence, through continuous education, training, research and reflective practice to support both the development of your team and the overall improvement of patient care. To develop as an advanced clinical practitioner and contribute to the on-call rota. About You: You will be a compassionate, experienced nurse with a deep understanding of palliative and end-of-life care. With excellent leadership and communication skills, you will inspire and guide a multidisciplinary team in delivering high-quality care. You will bring innovative ideas and a commitment to continuous improvement, ensuring that the services we offer are responsive, compassionate, and in line with the latest guidelines and best practices. Key Responsibilities: Lead and manage a team of clinical nurse specialists, fostering a supportive and collaborative working environment. Ensure the delivery of high-quality, evidence-based care to patients and families, respecting their wishes and preferences. Work closely with healthcare partners, including primary care teams, social services, and specialist providers, to ensure integrated care delivery. Develop and implement education and training initiatives to build clinical expertise and improve service delivery. Champion a culture of learning and reflective practice, ensuring continuous professional development for yourself and your team. Qualifications Skills Knowledge and Experience

Registered General Nurse with a valid current registration and experience of working at a senior level. Non-medical nurse prescribing qualification. Teaching and assessing qualification and/or extensive teaching experience. Commitment to develop as an Advanced Clinical Practitioner in Specialist palliative care. Experience in auditing clinical practices, ensuring compliance with regulations and hospice standards, and leading quality improvement initiatives. Disclosure and Barring Service Check
