The Advanced Clinical Practitioner will undertake assessment, diagnosis, treatment / referral and discharge of patients with primary / urgent care needs within their clinical competence. The Advanced Clinical Practitioner will provide clinical mentorship and leadership to all trainee and peer practitioners within the workplace when required. Job responsibilities

KEY PRINCIPLES Clinical Independently prioritise, assess, treat, and safely discharge/refer patients attending with undifferentiated and undiagnosed urgent primary health care problems. Undertake a range of clinical consultations via a range of modalities primarily face to face but may include telephone and home visits (service specific). Act as a referral agent and care co-ordinator by establishing multi-agency collaboration across the primary/secondary interface to meet the needs of the patient. Work within the parameters of current prescribing legislation or approved Patient Group Directives. Provide professional and clinical advice and direction to colleagues within your team. Maintain accurate, contemporaneous records. Research and Quality Demonstrate evidence-based care, working within agreed guidelines and protocols. Participate in audit, data collection and activity monitoring to improve performance and inform future service developments. Proactively report patient safety incidents in line with NHS's Just Culture and respond to Local Care Direct's Quality Teams requests for information during incident investigations. Take an active role in the management of safety incidents and organisational learning. Management Actively promote integrated working in collaboration with other health care professionals both within and outside the LCD organisation. Contribute to the development, implementation, review and evaluation of clinical protocols. Ensure effective communication, participating in meetings and liaising with other professionals, services and agencies as required. Professional and Educational Develop own skills and competencies through education, training and development activities related to the current and future demands of the role, seeking opportunities to enhance skills, undertake annual internal performance development review (PDR), in order to maintain current Registration and achieve Revalidation standards. Work within codes of conduct and practice set out by relevant professional body. Comply with the corporate governance structure in keeping with the principles and standards set by the organisation. Evidence of a range of clinical expertise, working independently in a healthcare role. Experience of working in an autonomous practitioner role. Evidence of competence in the clinical assessment, diagnosis and management of adults and paediatrics presenting with undifferentiated diagnosis. Experience of managing minor illness. Experience of mentorship, leadership and research in clinical practice. Experience of working in primary/urgent care. Emergency Care. Qualifications
One of the following: NMC Registered General Nurse OR BSc in Paramedic Science / IHCD Paramedic qualification OR BSc in Physiotherapy OR MPharm AND Advanced Clinical Practitioner MSc/Post Grad Diploma in advanced clinical practice (minimum 1 year post Masters study). Current registration with HCPC, NMC or GPhC. Spotting the sick child. Level 7 Minor Injury Qualification. MSc or PGdip in advanced practice (or equivalent (mapped to all outcomes on the Multiprofessional advanced practice framework HEE). HEE digital badge.
