Job Summary

To work as an autonomous practitioner, responsible for the delivery of acute care for patients via face-to-face, video, and telephone consultations and when required, home visits, delivering care within their scope of practice to the entitled patient population.

Main Duties of the Job

The candidate will need to demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder is an experienced nurse who, acting within professional boundaries, will provide care to patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care, managing acute daily demand.

To work with the practice team to meet the needs of the patients, supporting the delivery of policy and procedures.

The Advanced Practitioner will: Practice, without direct supervision, as an independent healthcare practitioner within various clinical environments, participating in care of all age ranges and presenting conditions on a daily basis. Have a mixture of acute on-the-day and routine appointments. Carry out home visits for housebound patients. Person Specification

Experience

Essential Experience of working in a primary care environment Experience of working as a practice nurse or community nurse Experience of chronic disease management Experience of prescribing and undertaking medication reviews Desirable Experience of working in a GP practice Qualifications

Essential Advanced Practitioner MSc/Post Grad Diploma with evidence of clinical competencies sign off. Level 7 Module of Advanced Clinical Assessment and Management (30 credits). Consultation skills. Prescribing. Desirable Experience in chronic diseases Personal Qualities
