Job Summary
We are looking for an Advanced Clinical Practitioner (ACP) to work as part of a new team at Ranworth PCN. Ranworth PCN is part of the national PCN pilot looking at new ways of working in primary care. We welcome applications on a full and part-time basis.
Main Duties of the Job
In line with the Fuller report, the proactive ambulatory care model is part of a broader shift within the NHS toward more community-based, patient-centred care that aims to keep individuals out of hospitals and support them in managing their health conditions more effectively at home.

Ranworth Primary Care Network is comprised of Caradoc Surgery, Ranworth Surgery in Clacton-on-Sea, Green Elms Health Centre in Jaywick, and Kennedy Way Medical Centre in Holland on Sea. We have a diverse patient population of around 32,000, and all practices work on SystmOne.
