Are you an Advanced Clinical Practitioner (ACP) with a passion for supporting patient care, who is keen to make a difference?

Stockport NHS Foundation Trust is currently offering an exciting opportunity for an ACP to join our community-based Urgent Community Response (UCR) and Virtual Ward (VW) Team(s). The multi-disciplinary (MDT) team consists of GP's, Advanced Clinical Practitioners, Nurses, Occupational and Physio Therapists, Therapy Assistants and Senior Support Workers.

UCR and VW work collaboratively to ensure patients receive safe, effective treatment at home, preventing the need for hospital admission. With support from our MDT, you will complete medical assessments in patient's homes to ensure appropriate intervention and treatment plans are in place to improve and stabilise their condition.

As a confident communicator with strong organisational skills and a passion for continuous development, you will be instrumental in supporting and improving a high-quality service. This will include development of new care pathways and involvement in a variety of projects where you will receive regular supervision, in-service training and support with the guidance of our ACP Service Lead and Nurse Consultant.

Main duties of the job

The post holder will be responsible for providing advanced clinical care and expertise in care delivery including judgement, discretion and decision making to patients as per individual scope of practice and through the advanced clinical competency framework.

Job responsibilities

Work autonomously as an ACP within the specialty, managing a caseload of patients and clinical priorities according to agreed protocols and working practices. Deliver individualised patient care, with direct responsibility for assessment, examination, investigation/ diagnostic procedures, and diagnosis of patients with undifferentiated and undiagnosed presentations. Appropriately treat patients, resulting in the safe management and appropriate onward referral or discharge of patients at the end of the care episode. Receive and triage referrals via a variety of sources, including direct patient referral. Participate in ward rounds, patient reviews, and multidisciplinary team meetings. Utilise scope of advanced practice to undertake Non-Medical Prescribing role and advise on medicine management issues associated with the patient group, working within Trust policy for Medicines Management.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary.

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Post registration experience at a senior level (band 6 or above) in community or a relevant speciality area Evidence of on-going CPD Evidence of teaching in a variety of environments Ability to work independently. Experience of using multidisciplinary approach to patient care
