Howdale Group Practice are looking to welcome an advanced nurse practitioner to join the team on a full time basis for 37.5 hours per week. Main duties of the job

The delivery of highly effective medical care to the entitled population. Manage patients presenting with a range of acute and chronic medical conditions, providing assessment, diagnosis, and treatment. Work collaboratively with the team to meet the needs of patients and support policies and procedures. Effective management of long-term conditions. Maintain accurate clinical records in conjunction with good practice, policy and guidance. Processing of administration in a timely manner, including referrals, repeat and acute prescription requests, test results, and other associated administrative tasks. Contribute to promoting and successfully implementing continuous improvement, development, and maintenance of the practice including clinical governance and training. Ensure compliance with the appraisal process and completing the revalidation process. About us

We are a 3 partner GMS practice with a list size of approximately 7,900, which is growing. We are based across two sites, Downham Market and Marham, both sites are dispensing. Qualifications

Be fully qualified and a registered nurse with the Nursing and Midwifery Council. Have a master's degree/postgraduate diploma or degree for Advanced Practice Qualification. Hold a qualified Independent nurse prescriber role on the NMC register, with experience of prescribing. Experience

Hold a long-term conditions management qualification. Hold a minor illness qualification. Have experience of working in a primary care environment. Be able to understand the needs of the patient and be sensitive and empathic. Be committed to supporting all the team and contribute to the development of the practice. Wish to continue with ongoing professional development. Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £50,000 to £55,000 a year depending on experience.
