To provide a high-quality, cost-effective service to the patients of the Practice; focusing on frail and older people, and those with low-level mental health concerns. You will also work with and triage other patients outside of frail and elderly. You will be working as part of a multi-disciplinary team which includes GP Partners, GPs, Advanced Clinical Practitioners, Nurse Practitioners, Paramedic Practitioners, Clinical Pharmacists, Physiotherapists, Practice Nurses, and Healthcare Assistants. You will be responsible for face-to-face and telephone consultations with patients presenting with a wide range of minor illness, urgent and same-day requests for treatment and chronic disease management, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. You will need to demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills. You will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. A large percentage of the role will be involved in the care of frail and elderly patients, including the responsibility of patients within care and nursing homes. This will include weekly ward rounds. Part of your responsibilities will include visiting patients in their home to assess their medical condition and frailty review. Experience

Experience of working with frail and older patients Experience of providing specialised advice in relation to care Two+ years proven record of working as an ACP dealing with patients presenting with a wide range of medical issues Experience of dealing with patients with undifferentiated and undiagnosed conditions Previous experience in General Practice Qualifications

Full and unrestricted NMC (UK) or HCPC registration RGN (degree or equivalent) Prescribing Qualification Qualification in Physical Assessment skills and Frailty A qualification in Frailty Care Knowledge and Skills

Evidence of continuing professional development Evidence of clinical knowledge in caring for patients in relevant specialism In-depth knowledge of recent NHS legislation, recommendations for specialist area. Knowledge of EMIS web Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £51,328 to £61,671 a year. HRH ordinarily appoint at the bottom of the bracket.
