Job summary We are looking for self-motivated inspirational individuals to work within the developing Intermediate Care Service. If you have a strong background in Domiciliary Care, Residential Care, Reablement or Rehabilitation in the community then this opportunity could be for you. Main duties of the job Over the past year, the previous Reablement Service has been developed into the current Intermediate Care Services. This role offers the post holder an opportunity to help design and mould the service delivery as it continues to transform. The post holder will need excellent communication skills with an underpinned knowledge of health and social care systems. Previous experience of supporting change would be advantageous along with a passion to develop person-centred restorative care in the community. The exciting opportunity which has arisen is for the position of Advanced Clinical Practitioner (Registered Nurse/AHP) within the Intermediate Care Service which is based at different localities across the Island, providing Island-wide services and is reportable to the Operations Team at Nobles Hospital. We would be willing to consider someone with Band 7 experience, who is training and looking for a development opportunity to the Advanced Clinical Practitioner role.
