Our Same Day Emergency Care (SDEC) clinics within Older People's Services are currently based on the Poole and Bournemouth Hospital sites and are at the forefront of delivering timely and effective interventions, to ensure our patients receive the best possible outcomes. We pride ourselves on delivering comprehensive, compassionate and patient-centred care for each individual and are seeking a dynamic and experienced Advanced Clinical Practitioner (ACP) to join our multidisciplinary team.
The ACP will play a pivotal role in the same day emergency care clinic, providing high-level clinical expertise and delivery of high-quality care to older people. The successful candidate will be responsible for managing complex cases, undertaking comprehensive geriatric assessments, ordering and interpreting investigations, and formulating patient-focused management plans.
The successful post holder will hold an accredited MSc in Advanced Clinical Practice or have completed the e-portfolio route with digital badge and be willing to complete our local competency booklet evidencing proficiency in the care of Older People.
This role requires an active UK NMC Registration.
