The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide autonomous healthcare to patients in Healthier West Wirral. The ACP is accountable for independent, competent clinical decision-making including assessment and diagnostic skills, within practice. In addition to this, the ACP is also responsible for appropriately referring patients to relevant colleagues or specialities for any necessary follow-up, inpatient, or outpatient review and/or investigations. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will be a registered healthcare professional and hold an MSc in Advanced Clinical Practice or equivalent at MSc level. They will be an independent Non-Medical Prescriber registered with their relevant professional body. They will practice at an advanced level demonstrating in-depth knowledge and competence in all aspects of acute, community, primary, social care medicine, encompassing advanced assessment, critical thinking and clinical management skills that are evidence-based and deliver high-quality patient-centered care. The post holder will support our 5 GP Practices with the provision of additional patient access on Saturdays, working one in eight at an enhanced rate of pay with a day off in lieu the week prior to the Saturday shift. Job Responsibilities (not exhaustive) Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Undertake house visits to frail, housebound, vulnerable and those otherwise unable to attend practice. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Prescribe and review medication appropriate to patient needs and in accordance with national and practice protocols, and within scope of practice. Work with patients to support compliance to prescribed treatments or over-the-counter medication. Support and manage health needs of women presenting for family planning, cervical cytology, or sexual health consultation. About Us

Manage own caseload and clinical priorities according to agreed protocols and working practices. Participate in multidisciplinary clinics, participate in ward rounds, patient reviews, and multidisciplinary team meetings.
