Job summary Scarborough Medical Group has an exciting opportunity for an Advanced Clinical Practitioner/Nurse Practitioner or Paramedic to join the team. We are a successful and highly regarded GP training practice based in the beautiful coastal town of Scarborough on the North Yorkshire Coast. With a patient list size of 15,900 patients, Scarborough Medical Group operates from two surgeries to the North and South of the town. We are a highly supportive training practice with a long-standing team including GPs, Advanced Clinical Practitioners, Nurse Practitioners, Practice Nurses, HCAs and many more additional roles. Training practice supporting specialty training, med students, registrars, student nurses and apprenticeships Twice daily clinical team catchup (20 minutes each) Mix of phone, online and face-to-face consultations Mix of acute and pre-booked appointments with a wide spectrum of work High Achieving QOF Practice Rated Good by CQC Friendly, supportive, innovative practice Free parking Main duties of the job We aim to ensure that the highest levels of care are delivered to all our patients, particularly in areas such as minor illnesses and acute, on-the-day appointments. The main duties of the role are to assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness and within the boundaries of professional qualifications. The post holder will use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment. To be considered for this opportunity you should meet the following: Registered NMC Nurse, or hold a current HCPC registration or registered ACP Minor Illness Qualification Masters degree level in relevant area of expertise, or equivalent level of experience Excellent communication and time management skills Full driving licence Ability to travel to other sites and perform visits in the community Ability to work occasional evenings and Saturdays with advanced notice and in rotation. We enjoy full GP and nursing team support including advanced clinical practitioners, nurse practitioners, specialist practice nurses and healthcare assistants. The patient support team and clerical teams work hard to support our clinical teams and patients. We are part of a strong, collaborative Primary Care Network and enjoy good working relationships with colleagues in the other Practices as a result. Job responsibilities The Clinical Practitioner will be responsible for providing routine and acute medical services to patients who wish to be seen by a healthcare professional. You will assess, diagnose, treat, and write prescriptions as required. You will triage and refer patients appropriately, making any necessary referrals for investigations in the appropriate manner including to Secondary Care. Primary responsibilities Assess patients presenting with acute illness. Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis within the boundaries of competence. Evaluate patients' medical conditions and determine the appropriate medications to prescribe. Clinically examine and assess patients' needs from a physiological and psychological perspective, and plan clinical care accordingly. Undertake home visits, primarily for acute conditions, as assessed by the duty doctor, and within the boundaries of professional qualifications. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs or liaise on patient care with the most appropriate member of the clinical team as required. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition. Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan. Ensure continuity of care, arranging follow-up consultations or reviews as necessary. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice). Liaise with external services/agencies to ensure that the patient is supported appropriately. Ensure adherence to the relevant patient group directives and local clinical pathways at all times. Maintain accurate clinical records in line with legislation. Fully participate in the clinical administration within the practice. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs. Work with patients and their carers to support compliance with prescribed treatments. Provide information and advice on prescribed or over-the-counter medication. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations. Support patients to adopt health promotion strategies that promote healthy lifestyles. Support and manage the needs of patients presenting for family planning or sexual health consultations. Assess, identify and refer patients presenting with mental health needs. Contribute to the practice achieving its quality targets. Help develop and set up new patient services and participate in initiatives to improve existing patient services. Attend the review of significant and near-miss events meetings. Ensure you are up to date with the latest local and National clinical guidance. Secondary responsibilities Support the Data Quality and Patient Support team managers with patient/process improvement initiatives. Partake in audits when necessary. Support and participate in shared learning across the Practice and with PCN colleagues. Take personal responsibility for own learning and development.
