An exciting opportunity has arisen for an Advanced Clinical Specialist Falls Lead within community.

The role is a 12 month fixed term position, involving working on an ongoing quality improvement project, targeting patients who have fallen or are at risk of falling, but currently do not meet referral criteria for services such as community rehabilitation and therefore, would not routinely receive a multifactorial falls assessment.

Hours per week: 30 hours

Main duties of the job

The role will work collaboratively with the falls lead in Mid Cheshire to continue project based work, commenced by the existing post holder. The role will also offer Advanced Clinical guidance to care community therapists across East Cheshire and will work closely with multi disciplinary team colleagues to offer falls training and support to care homes across East Cheshire to improve quality care.

Upon completion of the project over the next 6 months, the role will then expand and embed the new falls pathways across East Cheshire Care communities. The role will adopt a more clinical focus with a continual drive for service development and quality improvement.

Qualifications

Essential

HCPC (Health and Care Professions Council) registration Relevant professional degree Master's degree level or equivalent experience Professional/clinical knowledge acquired through degree supplemented by specialist training experience in specialist fields and practical experience to postgraduate diploma level or equivalent Membership of a relevant professional body & related specialist sub-groups

Knowledge

Essential

Post registration knowledge which is relevant to the specialist area In-depth knowledge of recent NHS legislation, recommendations for specialist area Detailed awareness of the national and local priorities and changing trends in the specialist area

Skills

Essential

Ability to lead junior colleagues Highly advanced communication and listening skills Ability to make appropriate decisions at all levels Audit skills Ability to prioritise workload, achieving a balance between clinical and other aspects of role Ability to work autonomously and as part of team Ability to work under pressure Proactive, takes own initiative Supportive team member

Proven post registration experience, in the relevant specialism Experience of planning and completing audit & research Experience of teaching and mentoring of staff Experience of undertaking projects

Prior experience working in the NHS - with a broad range of NHS physical rotations Proven ability to manage complex rehabilitation issues including dementia and ensure safe and timely discharge of patients Commitment to specialism as evidenced by CPD (Continuing Professional Development) and attendance at special interest groups
