To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways). Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice and PCN population. Assess patients presenting with acute illness or for review of chronic disease conditions. Diagnose, plan, implement and evaluate treatment/interventions and care for patients. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Fully participate in the clinical administration within the practice/PCN, signing prescriptions, reviewing medication, reviewing patient letters/discharges, respond to queries as required. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies. Refer patients as appropriate to other members of the PCN multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray. Undertakes and interprets a range of diagnostic tests and routine clinical procedures e.g., cytology, venepuncture, blood pressure monitoring etc. Makes critical judgements of the highest order to satisfy the expectations and demands of the job.
