Advanced Nurse Practitioner
Opportunity available for an Advanced Nurse Practitioner to work within Urgent Care, based in Harlow.
The team sits within an Urgent Care Centre, with the role predominantly responsible for the delivery of evidence-based nursing services. You will work as part of the service multi-disciplinary team, delivering care within your scope of practice to the entitled patient population. You will be working autonomously, and as a Nurse Prescriber, you will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, and well man clinics. You will also support the management team in reviewing clinical policy and procedure.
You will have access to appropriate clinical supervision and a named individual in the organisation to provide general advice and support on a day-to-day basis. As part of this role, you will develop, implement, and embed health promotion and wellbeing programmes, as well as evaluate individual treatment plans for acute and non-acute conditions. You will support the clinical team in developing individual treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
Providing routine nursing care to patients as required in accordance with clinical-based evidence, NICE, and the NSF is essential. You will be able to prescribe medication in accordance with PGD and within your own scope of practice, and review medication for effectiveness, need, and national guidance.
The hours for this role are Days / Evenings & Long Days
