A wonderful opportunity has arisen for an Advanced Nurse Practitioner to join our friendly rural practice team located 12 miles south of Lincoln. You will work within our highly valued and respected multidisciplinary team enabling us to provide and maintain excellent care for our patients. You will be an enthusiastic and motivated ANP. The successful Advanced Nurse Practitioner will work as an autonomous practitioner, diagnosing, managing and prescribing for patients with illness or injury. This may be by face-to-face, telephone, online or video consultation. We offer a competitive salary. We would be keen to support additional skills or areas of interest. In accordance with the practice timetable, as agreed, the post-holder will be available to undertake a variety of duties, including patient consultations, occasional home visits and dealing with queries, and clinical administration. Main duties of the job

Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action. Use complex decision making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice. Complete the relevant training in order to provide multi-professional clinical practice. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Act as an advocate for patients and colleagues. The post holder will have the appropriate level of child protection and adult safeguarding knowledge, skills and practice required for the post; be aware of and comply with child protection and adult safeguarding policies and procedures. This post will be subject to a DBS (Disclosure and Barring Service) Check. In addition to observing the host Practices rules and procedures, which all staff are required to observe and follow, the post holder is also required to follow general policies and procedures that apply to the employment relationship. GMS practice with a list size of 7100 patients. Professional multidisciplinary team comprising: 4 GPs, 2 ANPs, 2 Practice Nurses, 2 HCAs also supported by ARRS staff including clinical pharmacist, first contact physiotherapist, occupational therapist, mental health nurse, pharmacy technician, care coordinators and GPA. Provide care for presenting patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. You must demonstrate safe, clinical decision-making and expert care, including assessment and diagnosis skills for patients within the general practice. You will work collaboratively with the general practice team to meet the needs of the patients, supporting delivery of policy and procedures and providing nurse leadership as required. Experience

Working autonomously in an advanced nursing/clinical role. Nurse prescribing. Experience of working with a primary care setting or general practice. Experience in clinical assessment and management of a wide range of minor illness in patients of all ages. Qualifications

MSc Advanced Clinical Practitioner qualification. Registered Nurse. Clinical leadership skills. Experience of complex case management. Working with community development initiatives. Disclosure and Barring Service Check
