Proactively identifying, diagnosing and managing treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnosing and managing both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Prescribing and reviewing medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Be a registered first level nurse, MSc or equivalent, holding current registration with the NMC. Hold an independent prescriber qualification. Hold a mentor/teaching qualification. Have clinical supervision training. Have advanced clinical practice skills. Be able to work with a high degree of autonomy. Be confident in leading in a variety of settings: your leadership of others will also enable you to use and further develop your teaching and mentoring experience/qualifications. Be forward-thinking and will strive to develop innovative ideas and new ways of working, that address the health needs of the practice, whilst motivating and encouraging others to do the same. Be an excellent communicator and collaborator.
