You will act as an autonomous practitioner working both independently and in conjunction with other healthcare professionals to assess, diagnose and manage acute and long term conditions. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within the scope of practice. Make referrals as necessary to other members of the primary and secondary health care teams as appropriate. Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc. Support and advise patients, families, and carers with your clinical expertise. Recognise and manage palliative and end of life situations. Support best practice by working closely with the individual, their carers and family. Continuity is key to safe, effective and person-centred care, so establishing productive professional relationships with care home staff and knowledge of residents over time will support the delivery of excellent care.
