Credas medical are looking for an Advanced Nurse/Clinical Practitioner to join our team, to help deliver high-quality, person-centred healthcare to our registered patients in five care homes across Darley Dale.
Your role supports the care of residents in the care homes aligned to the practice, where your skills for the care of older people with frailty will be highly valued.
Based at Darley Dale Medical Centre you will work closely with our GPs and multidisciplinary team to care for patients in local Care Homes.

Main duties of the job
You will act as an autonomous practitioner working both independently and in conjunction with other healthcare professionals to assess, diagnose and manage acute and long term conditions. Undertake weekly visits to our five local care homes and provide an assessment of our registered patients' health care needs and advance care plans, based on highly-developed knowledge and skills, including skills such as physical examination and evidence-based prescribing. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within the scope of practice. Make referrals as necessary to other members of the primary and secondary health care teams as appropriate. Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc. Support and advise patients, families, and carers with your clinical expertise. Recognise and manage palliative and end of life situations. Support best practice by working closely with the individual, their carers and family. Communicate well with health and social care teams to ensure high quality personalised care including the completion of Respect and Advance Care planning documentation. About us

Continuity is key to safe, effective and person-centred care, so establishing productive professional relationships with care home staff and knowledge of residents over time will support the delivery of excellent care.
