Advanced Nurse Practitioner - Mountain View Health Centre

Join our friendly team as a Part-Time Advanced Nurse Practitioner at Mountain View Health Centre. The hours are Monday to Friday, 30 hours per week, 6 hours per day. The Advanced Clinical Practitioner (ACP) plays a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate. The ACP operates within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners. Key Responsibilities: Clinical Responsibilities:

Work independently within the scope of practice to manage complete and undifferentiated clinical episodes. Case Management:

Manage patient cases from initial assessment to conclusion.

Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care. Prescribing:

Utilise prescriptive authority responsibly following current guidelines and protocols. Leadership and Guidance:

Mentorship:

Provide mentorship and supervision to nursing staff and other healthcare professionals. Service Development:

Lead the development and implementation of new clinical services and pathways. Professional Development:

Engage in continuous professional development.

Research and Quality Improvement:

Experience

Experience of working within a nurse team. Previous experience in a GP surgery/Primary care in the UK, with a focus on managing long-term conditions and minor illnesses. Qualifications

Registered General Nurse (RGN) with current NMC registration, a recognized ANP/ACP qualification, and an Independent Prescriber qualification. Disclosure and Barring Service Check
