We are a GP surgery providing both primary and secondary care services. This role is working within the primary care service. A position has arisen for an enthusiastic and motivated individual to undertake the role of Advanced Nurse Practitioner. Applicants should be NMC Registered with Independent Prescribing. They should have a recognised Nurse Practitioner qualification working at degree level and be knowledgeable to lead in QOF, WPPC and Care Home work for example. The role is full time but there are options for working these hours over 4 days. There is also the possibility of management opportunities. Main duties of the job
Experience in management of minor illness, minor ailments, and injuries and chronic disease management. Participate in patient telephone and/or video consultations to manage patient demand and advise patients in the safest and most appropriate way relevant to their problems. Demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. Demonstrate critical thinking in the clinical decision-making process. Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. Job responsibilities
Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Manage caseload of housebound chronic disease management patients. Support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions. Telephone triage as appropriate for clinics and home visits and provide access to GPs as necessary. Undertake appropriate home visits. Take an active part in QOF management. Prioritise health problems and intervene appropriately to assess the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care. Advise, support and administer vaccinations for patients travelling abroad, where appropriate. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation. Triage patients (including Telephone triage) to undertake nurse-led clinics e.g. CHD, Warfarin/Anti-Coagulation, Diabetes, Rheumatology, Asthma and others as directed. Undertake Audits, Appraisals, Health Promotions and Teaching/Mentoring Staff. Implement and participate in vaccination programme as appropriate. Fit and remove implanon. Initiate insulin and manage patients to L4. Person Specification
