Advanced Occupational Therapist - Home First Service

An exciting opportunity has arisen to join the Home First Therapy team in Buckinghamshire. The team is still in its infancy of becoming a substantive service and this provides team members with the opportunity to influence and contribute to growing and shaping the service. We are looking for motivated and skilled individuals to join the team, which supports patients that have recently been discharged from hospital, to regain their independence and remain in their own homes. The ideal candidate will be able to take a holistic and person-centred approach to supporting patients to recover from their recent hospital admission, and to achieve their short-term goals, maximising independence. The role will include working with other Advanced Therapists in the team to effectively manage a complex caseload and see patients within the expected timescales. Additionally, the successful candidate will be required to support, teach and supervise Therapy staff within the team, to ensure that high standards and efficiency are consistently maintained. Main duties of the job

To be responsible for jointly leading and developing the Home First Therapy team, in regard to all aspects of clinical governance within this specialty. To perform specialised assessment and treatment for patients with highly complex presentations and needs, and to provide expert advice, guidance and information to health and social care professionals, carers, relatives and others. To undertake all aspects of clinical responsibilities as a skilled, autonomous and advanced practitioner. To actively lead development of professional and service initiatives, and contribute to organisational objectives. To supervise, teach and appraise Therapy staff and students within the specialty. Qualifications

Current registration with Health Professions Council Evidence of CPD in the form of a detailed personal development portfolio Membership of professional special interest group Field work/clinical educators course Experience

Minimum of 3-4 years working experience at Band 6 level in the NHS Attendance at post graduate courses in related fields Experience in supervision of junior staff Knowledge and application of evidence-based practice and clinical reasoning Ability to work and interact effectively within an interdisciplinary team Evidence of commitment to continuing professional development, and reflective practice Skills

Awareness of limitations of own professional abilities Clear, concise verbal and written communication to ensure liaison with multi-disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English) Ability to integrate, participate and contribute as an effective member of a team Ability to prioritise tasks and functions against well-reasoned standards. Ability to plan, manage and organise required tasks within available resources (including time) Use of and/or knowledge of POMR record keeping Knowledge of current related legislation (Care Act, H&S, Data P. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £46,148 to £52,809 a year per annum pro rata.
